The GLP-1 evidence base is, by peptide-class standards, unusually strong. That makes it especially important to distinguish what the trials actually showed from what is sometimes claimed online.

What the major trials established

• Robust glycaemic control in type-2 diabetes for multiple GLP-1 agents.
• Substantial weight reduction at higher doses in non-diabetic obese populations (e.g. STEP trials for semaglutide; SURMOUNT for tirzepatide).
• Cardiovascular benefit in defined high-risk populations for some agents.
• A characterised side-effect profile dominated by GI symptoms.
• Defined contraindications (e.g. personal/family history of medullary thyroid carcinoma).

What the trials did not establish

• Indefinite use beyond trial duration.
• Use outside studied populations (off-label aesthetic use, athletic use).
• Combination with unapproved peptide stacks.
• Use in pregnancy or in populations excluded from the trials.

Common online overreach

• “Easy weight loss” framing that ignores GI side effects, rebound after cessation, and individual variability.
• Endorsements that omit the prescribed-medicine context.
• Compounded or unauthorised semaglutide / tirzepatide sources presented as equivalent to the licensed product.
• Stacks combining GLP-1 agents with other unapproved peptides — not what the trials studied.
