Translational risk is the probability that a finding established in a cell line or animal model does not reproduce in humans. Across drug development as a whole the failure rate is high, and for peptide-class compounds it is particularly high because species differ in receptor distribution, immunogenicity, and pharmacokinetics. A compound is rated low only when it has UK-approved human evidence for the specific use described; moderate when there is some human data but it is limited or indirect; high when the evidence base is predominantly preclinical.
